Apple today began a voluntary recall of the Ultracompact USB Adapter, in a statement apple said “certain conditions the new ultracompact Apple USB power adapter’s metal prongs can break off and remain in a power outlet, creating the risk of electric shock.”

The adapter is for the 3G iPhone, and thankfully according to Apple no one has been hurt.
